A GCM is a professional that assists with addressing the needs of the elderly population. A GCM assists in the process of planning, monitoring, linking, and coordinating care in order to meet the needs of the elderly & maximizing life.

Benefits of having a GCM include saving money, making better decisions, navigation of health systems, supervision & advocacy, & caregiver support.

Coverage depends upon health insurance plan. However, most commercial & private paid insurances do not cover these services. Additionally, Medicare recipients may be eligible for chronic care management services.

A GCM will explore family/social supports, home safety, spirituality, finances, nutrition, environment, mental status, & functional status.

Communication will be delivered via telephone, teleconference, secured email, secured fax, or traditional mail.
